<p>Non-stick vegetable cooking spray<br />
1/4 pound potatoes, peeled and sliced<br />
2 center-cut pork chops, well trimmed<span id="more-505"></span></p>
<p>1/8 teaspoon salt</p>
<p>1/8 teaspoon garlic powder</p>
<p>1/8 teaspoon onion powder</p>
<p>1/8 teaspoon black pepper</p>
<p>1 1/2 tablespoon water</p>
<p>1/3 cup mushrooms, sliced</p>
<p>1/4 cup onions, sliced</p>
<p>1/2 cup light sour cream</p>
<p>1 1/2 teaspoon fresh parsley, minced or 1/2 teaspoon dried parsley<br />
Heat skillet over medium heat. Spray with non-stick vegetable spray. Add potatoes and onions; brown lightly and set aside.</p>
<p>Season pork chops with salt, garlic powder, onion powder and black pepper.</p>
<p>Re-spray skillet. Cook chops for 1 minute on each side; drain fat.</p>

			<content:encoded><![CDATA[<p style="text-align: justify;">How often do you wish you had more answers? Or better answers? How often have you had to answer a client&#8217;s fitness question with, &#8220;well, it depends ?‰.&#8221; You launch into an explanation of the various possibilities, only to detect that glazed eyeball look &#8212; a polite but vacant stare that tells you they don&#8217;t want the whole dog and pony show, just a simple answer. After your workout. Four sets. Two glasses of milk.<span id="more-502"></span></p>
<p style="text-align: justify;">Perhaps you have felt frustrated yourself trying to make sense of conflicting headlines: Yes vitamins work, no they&#8217;re worthless. Or dealing with ambiguity: Something may work some of the time in certain situations for one group of people. Or frustrated by continually having to come up with the best advice possible with very little information.</p>
<p style="text-align: justify;">Scientific method or madness?<br />
The process of scientific inquiry is the source of both knowledge and frustration. The scientific method is a very important way of learning about reality. It begins with the idea that we can learn about the relationships among variables by studying them in a systematic fashion. Knowledge acquired from this method can then be used to make predictions about the future behavior of these variables and, possibly, intervene to alter this behavior.</p>
<p style="text-align: justify;">Sounds simple. So, where&#8217;s the madness? The frustration comes from several corners. One is the painstaking exactness with which extraneous variables must be controlled or accounted for. This means the scientific method is extremely time-consuming and expensive. Achieving an adequate measure of control often results in small sample sizes or homogenous subject groups, which then limit the applicability of a study&#8217;s findings.</p>
<p style="text-align: justify;">Researchers often look at only one aspect of a very large picture, so that their study seems to have little relevance to that larger picture. Studying one little piece of the puzzle can also lead to contradictory findings, so that in the early stages of research on a particular topic, scientists can seem like the blind men studying the elephant. In this parable, each man comes to a different conclusion concerning the nature of the elephant, depending upon which body part he feels. The man feeling the tail concludes the elephant is like a rope, whereas the man feeling the trunk concludes the elephant is like a snake, etc. In recent years, we have read that antioxidants don&#8217;t help prevent illness (over a relatively short time period in people who have smoked for many years) yet do help (in people with a high lifelong consumption of fresh fruits and vegetables).</p>
<p style="text-align: justify;">Another source of frustration is the communication gap between scientists and the public. Scientists write their studies for scientific journals whose readership is comprised of other scientists. The language of most scientific disciplines is quite dense and difficult to read without some practice and a specialized vocabulary. Science writers attempt to translate this information for the public, but information critical to evaluating the research can get lost in the translation so that general readers get incomplete information. Adding insult to injury, the popular press may then make a very sensational headline out of a marginally significant piece of research, or generalize a study&#8217;s findings beyond their true applications.</p>
<p style="text-align: justify;">Risky business<br />
Most frustrating of all is that even after many studies have confirmed, for example, the importance of a lowfat diet in preventing high blood cholesterol levels, variables do not always behave the way they are supposed to. Perhaps you have had clients who exercise, eat well, manage stress and still have high cholesterol levels. There are exceptions to every rule. Most of the time scientists give us odds rather than certainty. In the long run, on the average, a certain reduction in blood cholesterol level is associated with a certain reduction in risk of heart disease. But a given individual who substantially reduces his or her blood cholesterol level may still go on to have a myocardial infarction. For this person, odds are meaningless; if you have an MI, you have it 100 percent.</p>
<p style="text-align: justify;">Clients often forget the meaning of risk factors, that they are merely estimates of risk. These estimates are the best information we have, but they are not predictions, and certainly not promises. We each do the best we can to increase our &#8220;risk&#8221; of good health, but we never know what life has in store for us.</p>
<p style="text-align: justify;">Science in the fitness center<br />
Fitness professionals are respected by their clients because we have the inside track on health and fitness information. We are educated and we keep abreast of the latest research findings.</p>
<p style="text-align: justify;">Sometimes we get into a jam when at one point we recommend one thing, then research suggests we should start recommending something else. Readers who have been advising clients for many years may remember teaching them to stretch before their workout. Now we are advising them to stretch after a good warmup. Although most clients are sympathetic to the changing nature of health policy recommendations, some lose patience with the inexactness of health science. Their frustrations remind us of a few important points.</p>
<p style="text-align: justify;">1) Stay informed. You will be perceived as more knowledgeable because you are. Credibility is very important in any field. Read publications that help you stay in touch with research in health and fitness. Getting information from several sources allows you to compare conclusions to be sure you are getting a balanced view.</p>
<p style="text-align: justify;">2) Treat the conclusions of any given research study with respect, but not reverence.There is always a possibility that significant results occur due to chance. Folks in the position of making policy recommendations (that includes us) should be slow to adopt new advice. After many well-designed studies have supported the same conclusions, we can be more certain that those conclusions are valid.</p>
<p style="text-align: justify;">3) Be careful when telling people how to live their lives. Fitness professionals should follow the dictum to health-care professionals: &#8220;First, do no harm.&#8221; It is sobering to think that our well-intentioned recommendations can inadvertently lead to marital discord, financial problems or injury.</p>
<p style="text-align: justify;">4) Acknowledge that we simply don&#8217;t have &#8220;all the answers.&#8221; We can only do the best we can with the information we have. Our knowledge about human physiology is constantly growing. As it grows, theories change, and so do public health recommendations. Look at how much more we know now than 10 years or 20 years ago. Nevertheless, the fact that many questions remain unanswered means that we must live with uncertainty, which for some people is very uncomfortable.</p>

I have been told that I have numerous conditions, been prescribed numerous medications, undergone both conservative and invasive treatments and continue to be in constant pain. I am now told that I need more tests that I have had before.<span id="more-489"></span> I don&#8217;t know where to turn. I cannot afford all of this &#8220;medical care.&#8221; My problem was initially diagnosed as ruptured discs at L-4/L-5 and L-5/S-1. I subsequently had laminectomies and have had nothing but suffering ever since. I am almost completely immobile. I don&#8217;t know whether to commit to additional testing, continue taking pain medication, or consider more surgery. I can&#8217;t get any doctor to listen to me. My life is out of control. Can you offer any advice?</p>
<p style="text-align: justify;">L.L.</p>
<p style="text-align: justify;">A.Chronic back pain is one of the most difficult problems that I treat. Failed back surgery creates further difficulty in diagnosing the problem. In such cases, it is very difficult to determine if the exact cause of the pain is from recurrent disc herniation (bulging) or scar tissue. As a patient, it is very hard for you to know what questions to ask your physicians. Doctors often seem to be in too much of a hurry to listen.</p>
<p style="text-align: justify;">A. &gt;From your question, it is not clear what tests you have had and when. I often recommend a magnetic resonance imaging (MRI) of the spine. We do this with or without gadolinium, a type of dye that can help us differentiate between recurrent disc herniation and scar tissue. The importance of this is that scar tissue is very difficult to treat, and recurrent surgery to remove scar tissue does not often give very satisfying results. Another test that can be quite helpful is a myelogram. In this procedure, dye is injected around the spinal nerve roots, and then X-rays and a computerized tomography (CT) scan are taken. This test helps detect any pressure on the nerve roots, but it doesn&#8217;t differentiate disc from scar tissue.</p>
<p style="text-align: justify;">Unfortunately, your life has reached the point where normal activities of daily living have become difficult, if not impossible. It is hard for me to recommend any specific treatment based on your question alone. At this point, you need to get as much information as possible so that you can make an educated decision about treatment. A specialist in spine surgery would be the best person to handle your case. It is also important to choose a physician who is willing to spend time with you, answer your questions and go over all of the options. Hopefully, the cause of your problem can be found and corrected. If surgery is not an option, then you would do best to see a pain management specialist to try to lessen your symptoms and allow you to return to a more normal lifestyle.</p>

			<content:encoded><![CDATA[<p>While researchers continue searching for a cure for diabetes, others are focusing their time on medical and technological advances to improve the day-to-day lives of diabetics.<span id="more-478"></span></p>
<p>Lantus, the newest long-acting insulin on the market, is being touted for its convenience, mainly because it only has to be injected once a day at bedtime.</p>
<p>It can be taken by adults with type 2 diabetes who need long-acting insulin to control hyperglycemia and by adult and pediatric patients with type 1 diabetes, according to Aventis Pharmaceuticals, the New Jersey manufacturing company producing the insulin.</p>
<p>&#8220;It&#8217;s almost like [an insulin] pump, in that it provides a steady flow of insulin&#8221; for the entire 24 hours, according to Dr. Marian Parrott, vice president of clinical affairs for the American Diabetes Association. &#8220;It&#8217;s very promising for those who&#8217;ve had problems with hyperglycemia, which is a big problem,&#8221; she said.</p>
<p>&#8220;It has a constant concentration time of 24 hours compared with the 14.5 hour duration of most other products,&#8221; according to information on the Aventis Pharmaceuticals Web site. The insulin, approved by the U.S. Food and Drug Administration in April, is now on the market.</p>
<p>Another advance, though still being tested in clinical trials, is inhaled insulin.</p>
<p>An inhaling device, about the size of a cellular phone, delivers regulated dosages of insulin to the lungs, either in a mist or fine powder form. The patient takes a slow deep breath and inhales insulin into the lungs, where it is transferred to the blood stream.</p>
<p>Still, &#8220;It&#8217;s pretty short acting, so it would be more useful as a supplement to a long-acting insulin,&#8221; Parrott said.</p>
<p><a href="http://www.remedy4pe.com ">Also under development &#8212; though not as far along as inhaled insulin &#8212; is oral insulin, which would allow the person to take insulin in a pill form instead of having to inject it.</a></p>
<p>Currently, insulin must be injected into the fat under the skin to get into the bloodstream.</p>
<p>The ultimate goal, according to Parrott, would be an insulin pump that automatically monitors blood sugar levels, adjusts the amount of insulin and delivers it. &#8220;It would be like an artificial pancreas,&#8221; Parrott said. &#8220;Then you could go about your business, have a snack when you want, sleep late &#8230; without having to worry about having a bad reaction.&#8221;</p>
